	Pick up an entry form at the participating outlets and fill in your details.

	Attach 2 of your baby’s pictures (3R/4R-size close-up of baby’s face & full-body shot), proof-of-purchase and the entry form, and mail these to: “Johnson’s Baby of the Year 2008” Contest, Peti Surat Pejabat Pos 10472, 50714 Kuala Lumpur
